Acceptable Form of Identification?
As a notary public, I am required by law to positively identify all of my customers. Any of the following are acceptable forms of ID:
State Issued Driver’s License
Military ID Card
Government ID Card
PA Non-Driver Photo ID
Current Passport
Identification must include your photograph and signature. This is required for all buyers, sellers or individuals signing affidavits.

Notary
Any and all parties must be present with acceptable identification for document processing. All sections can be completed prior to the affidavit. Each person must be present and sign in the presence of the notary. Prior signatures cannot be notarized! If witnesses are required please bring them with you along with their proper acceptable identification. Witnesses should be an individual you personally know but who has no interest in the transaction.
Title Transfers (PA Titles) Required Documents
Title, seller and buyer with acceptable proof of identification, mileage, proof of insurance. Bill of sale should state seller name and address, buyer name and address, Year, Make, Model and Vin, Mileage and Purchase Price.
Title Transfers (Out-of-State) Required Documents
Title, seller or properly re-assigned title, buyer with acceptable proof of identification, mileage, proof of insurance, vehicle (for VIN verification) and bill of sale. Bill of sale should state seller name and address, buyer name and address, Year, Make, Model and Vin, Mileage and Purchase Price.

Renewals/Replacements of Registration
Requirements include valid photo identification, proof of insurance and renewal application or MV-140 are required. The MV-140 can be downloaded and completed prior to your arrival if you have not received a renewal application in the mail. This form can also be picked and completed here at the office.
Driver’s License Renewals/Replacements
Bring in your renewal form or download the DL-80. The DL-80 can be downloaded and completed prior to your arrival or this form can be picked up and completed here at the office. Once completed the application will be sent via Messenger Service for expedited processing. Please note that CDL Licenses can be completed here but do take and average of 3 – 5 days longer. All licenses that are renewed must be picked-up personally in the office and all old licenses must be given to the messenger service upon pick-up of new license.
Moving to PA?
You must go to a Drivers Exam Center near you with the following items: out-of-state license, social security card, birth certificate and two items proving your residence in PA. Once you have obtained a new PA license then you can apply for insurance and registration of vehicles in PA.
VIN Verification
VIN Verification is required by law for all vehicles coming from out-of-state. If you are unable to bring the vehicle to the office for verification then there are two acceptable proofs of verification:
- Make a complete pencil tracing of the VIN this must be legible
- Form MV-41 must be completed by a licensed PA Inspection Mechanic or State Trooper verify the VIN
Weight Ratings
Trucks and trailers must be verified on MV-41 by an Inspection Mechanic. This cannot be verified by a notary! Empty or unladen weights not listed can be verified using a weigh slip.
Suspensions
PA State law allows you 30 days to re-instate any lapsed insurance. If your insurance has been re-instated within 30 days registration can occur provided the following. You must complete the form MV-211 affidavit that you did not drive the vehicle within that time and provide proof of insurance that states the year, make, model, effective date, expiration date and insurance company name. If your insurance has lapsed for more than 30 days you are required to return your tag and begin a 90 day suspension of registration. After this time you must pay $50.00 for restoration and provide current insurance for registration.
Title Replacements
If you have lost your title complete form MV-38 O and sign in the notary presence. This must be notarized! This can be sent via messenger service or the customer can mail for processing. If there is a current lien which has been paid you are required to provide a release of lien letter from the lien holder.
Reconstructed Vehicles/Specially Constructed/Out-of-State Title(Salvage)/Modified
All of the above listed vehicles can be registered and titled in PA with the following method: Form MV-426B must be completed by an Enhanced Safety Inspection Station. PA Salvage titles can be registered here on-site. All other applications must be sent to PennDOT for processing either through using the messenger service or the customer can mail the application. Incomplete applications cannot be processed!
Apportioned Registration
Apportioned registrations can be completed here in the office for new accounts or fleet changes. Upon the completion of all forms you will be provided a temporary apportioned registration on-site. IFTA stickers and HUT registrations are also currently handled here.

Fleet changes must provide the following: driver’s license, proof of insurance, IRS 2290, title, lease agreement (if applicable) valid DOT # (in your name), valid proof of sales tax exemption either ICC or PUC (these are checked), verified weight (if applicable), transfer tag.
